Welcome to CROSSROADS PREMIERE HEALTH CARE, PC. I, Doc Sandy, appreciate the trust and confidence you have placed in my practice. In return I am committed to providing you with a breakthrough medical experience through opportunity, choice and hope.
I am very thorough and this takes time so be prepared to spend about 2 hours with us at your first visit. This allows me to get to know all the details of your life, explain my findings to you, make suggestions, answer your questions and help you develop a plan of care that will work for your specific circumstances and conditions.
Because my style of practice is to take time to serve you with caring and listening, I sometimes run behind schedule. Should this occur, please remember that when you need a little extra time, it will be there for you as well.
For follow-up visits, the time you are asked to be here is for check-in, not the time you will see me. Check in for Osteopathic Manipulation Treatment will be 30 minutes prior to when you are scheduled to see me.
I have a pet therapist named Inu (ee-new). She is a black lab/chow mix. If you have any problems with dogs, please alert the staff prior to your visit.
To assist you, we have enclosed the registration and health history intake forms for you to fill out. Please mail or drop them off (we have a drop box) at the office 2-3 days before your appointment. This will decrease your waiting time at your first visit. If you can’t drop them off or mail them back, bring them with you to your visit and please be prepared to enjoy a cup of tea, a magazine and music in our living room while you wait for us to enter your information into the computer.
